1) After completing the form you will receive via email an excel template that has all of the information needed for us to complete your 1099s. (Please check your spam or junk email for the email in case your do not see it)

2) Fill out the form carefully and double check all of the information.

3) Upload the form to your secure portal https://portal.progrowthsolutions.com
